Who pays, and what goes on each line
The invoice goes to the parent who pays. Put the dates, start and end times, and each child’s first name in the line description. Batching a month of regular afternoons on one line is fine as long as every date is listed. Writing line item descriptions has more on wording.
Worked example: one month of babysitting
- After-school care, 6 afternoons (Sep 8, 10, 15, 17, 22, 24), 3:15 to 6:15 pm: 6 × 3 hr = 18 hr at $20.00 = $360.00
- Evening care, Sep 19, 6:00 to 11:45 pm: 5.75 hr at $20.00 = $115.00
- Evening care, Sep 26, 5:30 to 10:00 pm: 4.5 hr at $20.00 = $90.00
Total $565.00, no tax. The family paid $120.00 on Sep 12 for Sep 8 and 10, and $235.00 on Sep 20 for Sep 15, 17, and 19. That $355.00 goes in Amount already paid, so the PDF shows Balance due $210.00: Sep 22 and 24 ($120.00) plus Sep 26 ($90.00). These are example prices, not typical rates. Use your own. The names are examples too.
Times, late nights, extra kids
- Write times as decimal hours with the hr unit: 15 minutes is 0.25 hr, 30 minutes is 0.5 hr, 45 minutes is 0.75 hr. More on rounding in hourly vs fixed fee invoices.
- A higher rate after midnight goes on its own line. “Evening care, Oct 3, 7:00 pm to 12:00 am” is 5 hr at $20.00 = $100.00, and “After midnight, Oct 4, 12:00 to 1:30 am” is 1.5 hr at $25.00 = $37.50. Together that is $137.50.
- An extra child can be one higher rate for the group or a separate line. Only bill what the family agreed to.
- A late cancellation can be a line like “Late cancellation, Oct 9 (under 24 hours notice)”, but only if you agreed on it in advance.
If the family needs it for a dependent care FSA
Other employers’ plans often ask for similar details, but check the family’s plan. For a dependent care FSA claim, FSAFEDS (the FSA program for federal employees) asks for either the provider’s signature on the claim form or an itemized statement from the provider, along with a completed claim form. The statement has to include:
- Service dates: every date is in the line descriptions.
- The dependent’s name: each child’s first name is on every line.
- Type of service: each line says after-school or evening care.
- Amount billed: each line has hours, rate, and amount.
- The provider’s name and address: they go in the From block.
FSAFEDS also says credit card receipts, canceled checks, and balance forward statements don’t meet its requirements. That is why every date is listed, not just a balance. Plans differ, so check what the family’s plan and claim form ask for. Date the invoice after the last day of care. Once it’s paid, click Mark as paid, keep Show PAID label on, and send that copy. Your taxpayer ID and Form W-10
The invoice has no field for a Social Security number, and you don’t need to put one on it. The family may still need your taxpayer ID (an SSN or an EIN) for their tax return, because families who claim the credit for child and dependent care expenses, or who get benefits under an employer’s dependent care plan, report each care provider’s name, address, and taxpayer identification number. The IRS says they may use Form W-10 to ask you for it. Nothing you type here is uploaded.
How to finish it
- Click “Start with the babysitting template”.
- Change the names, dates, times, hours, and rate.
- Enter what the family has paid in Amount already paid, with the dates in the notes.
- Download the PDF and send it.
- Next month, click Duplicate as new so the number goes up (SIT-0930 becomes SIT-0931), then replace the dates.
